The Cooperation packet is a required document for custodial parties who want to continue receiving Income Support Division (ISD) – Child Support Services Division (CSSD) services. ISD provides this document to custodial parties applying for TANF and/or Medicaid benefits when they have indicated on their Application for Assistance that one or both parent(s) of the child(ren) for whom they seek assistance, does not reside in the custodial party’s household. This document must be completed and returned to CSSD.
CSSD sends this document periodically for a custodial party to complete and return. When you receive one of these documents, please complete and return it to CSSD as soon as possible for caseworker review. The purpose of this document is to help establish parentage, a child support court order, and/or health care; to verify if the custodial party is receiving payments; and to verify if the custodial party has any new information that is relevant to the case, such as information for the child(ren) which was not known/provided when applying for services, or new information regarding the paying parents location or employer that CSSD may not be aware of. Failure to return this form may result in sanctions and reduction of other benefits from the ISD, and possible closure of your CSSD case.
