Co-operation Packet Information

The Co-operation packet is a required document for parents who want to continue receiving CSSD services. CSSD sends this document periodically for a party receiving payments to complete and return. When you receive one, please complete and return it as soon as possible for caseworker review. The purpose of this document is to verify if the parent receiving payments has any new information that is relevant to the case, such as information for the children which was not known/provided when applying for services, or new information regarding the paying parents location or employer that CSSD may not be aware of. Failure to return this form may result in sanctions and reduction of other benefits from the Income Support Division, and possible closure of your CSSD case.
