Provider Selection

As part of its mission, the Developmental Disabilities Support Division (DDSD) provides individuals, families, and caregivers with information needed to guide important life decisions. The following tools help gather information about service providers before making a selection.

    Provider Selection Guide

    The Developmental Disabilities Support Division (DDSD) understands that there is a lot to think about when choosing an agency for each service. This can include location of the service provider, experience, staffing, safety, and ability to access community resources. To help with this, DDSD developed the Provider Selection Guide, a tool for assisting people receiving services (and their guardians if they have one) to select the right provider for their support needs.

    Start here with the Provider Selection Guide Tool:

    1. With your Interdisciplinary Team (IDT) or Circle of Support, develop an Individual Service Plan or Service and Support Plan (depending which waiver you are receiving services through) based on what you want for your life, and the services and supports you need to make those things happen.
    2. Talk with your Case Manager or Consultant about what you should expect from each service provider. You can also review the Service Standards of the waiver you are getting services from.
    3. Access the DDSD Secondary Freedom of Choice to get a list of providers in your region or county.
    4. Try the DDSD Provider Selection Guide Interview Questions  to help you to make informed decisions about selecting an agency. These are suggestions; you can choose which questions you want to ask. You can also use space to write your own. (En Espanol: Preguntas de Entrevista al Proveedor)
    5. Contact the providers in your region or county who are accepting new clients. You can call and make an appointment to meet with them or make an appointment to meet by phone to talk about their services and ask questions.
    6. If you are not satisfied with the answers provided, you can ask to speak to a supervisor or the manager of the agency and/or look for a different agency.

    Data Summary

    The Provider Data Summary is a list that has data with information about service providers in New Mexico. DDSD wants you to have this information to understand the different services that providers may offer. It can also help you start a conversation with the interdisciplinary team, including the possible service providers, so you (the person in services and/or the guardian) can make an informed decision.
