YesNMConnect.gov is New Mexico’s statewide Closed Loop Referral System that connects New Mexicans to housing, food, transportation, behavioral health, and other essential services, and allows health care providers and organizations to track referrals and “close the loop.” Using YESNMConnect, we can make sure everyone is our state has access to the services they need to attain their highest level of health.

Phased Rollout with Shared Accountability
- 15 counties, state agencies, 4 managed care organizations, 988/211 networks, and UNM Health participating to date.
- More counties onboarding in 2026 through a phased approach with training, technical assistance, and sustainable funding—while generating county-level data to improve coordination, planning, and outcomes over time.
